Time of service payments and copays are due at the time of service.
*Please note: These are discounted prices from normal rates. These rates are charged to anyone paying at time of service and not requesting that we provide the service of insurance billing.

What to Expect
New Patient Visits include an in-depth health history intake and treatment plan development in addition to an initial treatment. Initial visits are scheduled for 75 minutes.
Follow-up visits are scheduled for 60 or 75 minutes.
A 60 minute treatment usually involves 5 - 10 minutes of conversation, 25 - 45 minutes of acupuncture (combined time for both insertion and resting with needles), 5 - 10 minutes of other therapies (moxa, cupping, massage, gua sha), Chinese herbal formula prescription if needed and desired, Diet, Lifestyle, and Supplement counseling and written plan if needed and desired.
Reasons to get an extended (75 minute) visit:
You have multiple areas of the body that need care in one treatment
You would like both front and back acupuncture segments, plus brief cupping, massage, moxa, or gua sha.
In addition to acupuncture, you would also like more than 10 minutes of cupping or massage
Dr. Eastin recommends it based on her recommended treatment plan, i.e. if she feels you would benefit benefit from more time & therapies.
You would like or think we will need more than about 10 minutes for conversation, yet would still like the usual amount of time for treatment (Dr. Eastin loves to talk with you but there is only so much time allocated to each treatment so more conversation time does cut into the treatment time)
You like to rest with the needles for more than about 30 minutes
Poultice application
If you know that you would like more than 60 minutes for your visit, you can schedule an extended (75 min) visit at the time of scheduling. This will incur an additional charge of $25 at the time of service, even if billing insurance, as insurance will not pay for this extra time. If you scheduled a 60 minute visit, but decide you would like an extended visit once you have arrived, it may be possible to extend your visit depending on that day’s schedule.

We accept Oregon Motor Vehicle Accident claims and workers’ comp. We are not able to accept OHP or Medicare plans. If you have an insurance not listed above, we may be in the process of credentialing with that company, so feel free to ask.
Once you have scheduled, if you would like us to bill your insurance, we will confirm your acupuncture and physical therapy insurance benefits. We do need at least 2 business days prior to your first visit to do this, otherwise we may not be able to bill your insurance for your first visit.
We also need YOU to confirm your current acupuncture and physical therapy benefits with your insurance company, in case they do not give us the correct information, which has been known to happen.
Although we do our best to keep up to date with your current visits used and details of your deductible as it applies to acupuncture, it is primarily your responsibility to be aware when you have reached your plan limits and whether your deductible applies and has been met.
The patient is responsible for payment out of pocket for any visits occurring before or after these limits have been reached. At times we do not receive correct or up-to-date information, thus you being aware of these details helps prevent financial surprises.
If your plan requires prior authorizations, this means that we have to submit thorough information to your insurance company after your first visit in order to find out if they will cover acupuncture and physical therapy for your current complaint. If this applies to you, we will provide you with more detailed information about this process.
Some plans provide acupuncture benefits, but only pay for 30 minute visits. Since Dr. Eastin’s treatments require at least 60 minutes, and involve services which are not covered by these plans (such as Chinese herbal formula prescription, cupping, moxibustion, bodywork, and diet and lifestyle counseling), we need to charge a fee for ‘Non-Covered Services’ when these plans are utilized.
This is the only way we can sustainably accept these insurance plans.
The Non-Covered Service fee is $25 for a 60 minute treatment, $50 for a 75 minute treatment (see above for more on 60 versus 75 minute treatments).
*Financial Hardship Discount
It is part of my purpose to make these healing treatments available to as many people as possible. Because I am a provider for many different insurance companies, I am required to use standards based on income in order to offer discounts.
We offer 60 minute visits for $90 (reg. $110) and 75 minute visits for $110 (reg. $135) to people whose Adjusted Gross Income (AGI), tax form 1040 on last year’s taxes is below $25,000 for one person, below $30,000 for two or more people. Please bring in a copy of the 1040 form of your tax return each year in order for us to provide this discount. We do not keep social security numbers on file.
Cancellation / No Show Policy
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly “full” appointment book.
Arriving 15 minutes or more late generally does not allow enough time for an appointment and thus will generally qualify as a late cancellation.
If any appointment is cancelled less than 24 hours in advance you will be charged a sixty dollar ($60) fee; this will not be covered by your insurance company.
